Most articles blame the absence of Bluetooth phones to lack of bluetooth capable chipsets from Qualcomm.
This is from TheRegister posted 06/2002:

When I looked around I read a news brief about Qualcomm unveiling the first CDMA200 1X Bluetooth capable chipset back in October 2001. cdma2000 1x bluetooth article
So i guess, Qualcomm can not be solely blamed for the lack on Bluetooth in cdma phones.
