If nobody answers, go to the place where you bought it, may be they can give you an advice. I know for sure that the carriers are giving unlock codes if somebody asks (and pays of course)

Default code is last 4 digits of phone number -- if you changed it and Verizon is your provider you may be able to re-reset it to last 4 digits of your number by dialing *228 and selecting option 1.
