a) its a CDMA phone
b) its of a model that appears on Verizon's approved list
c) the meid or esn is not on the blocked list
d) its not a pre-paid phone, or has been active on a pre-pay account for a minimum of 6 months.

Typically CDMA phones are NOT locked. So it all boils down to what the OP actually meant by an "unlocked phone"
